Information needed to port or transfer my number to a VoIP provider - How do I get it?

Hi,

Ii would like to transfer my phone number to JMP.chat and need to fill out data such as:

billing address (I don't think lucky has my address but would this mean the address when i setup my credit card?)

account number (how long is it supposed to be)

account PIN (does lucky have this)

I am very grateful for any help, thanks a lot!
